How to Enroll Your Student in the Boise School District

Secondary Grades 7 - 12

• The following forms and documentation are required:
  • A certified birth certificate
  • A record of the student’s immunizations or an immunization exemption.
  • A completed School Information and Enrollment Form
  • A completed Secondary Health Enrollment Form
  • If your child requires medication to be administered at school an Authorization for Medication Form is required.
  • A completed Home Language Survey
    The Home Language Survey is available (and many other forms) in multiple languages from our English Language Learners Forms page.
  • Proof of residence
  • If divorced, a copy of the divorce decree and custody agreement or any papers pertaining to current legal proceedings.
  • If you are a custodial guardian, you will need a "Power of Attorney" document, which must be renewed every six months.
  • Middle school (junior high)/high school transcripts.

Physical Examinations

The Board requires a physical examination (completed after May 1^st of 6^th grade year) to provide eligibility for physical education classes in grades 7-12 and athletics in grades 7-8.

Students participating in interscholastic athletics will be required to have physical examinations at the beginning of 9^th and 11^th grades. At the beginning of the 10^th and 12^th grade years, if the student has completed a physical exam in the previous year, the parent must complete and return an interim questionnaire. The exam is done at the expense of the student.

Release of Student Directory Information (FERPA)

The Boise School District complies with federal law (Family Educational Rights and Privacy Act) as it relates to the release of student directory information to the public. More information and parent/guardian form.

Concurrent Enrollment

Concurrent credit for specific courses taught in the Boise School District is available through Boise State University, Northwest Nazarene University and the College of Southern Idaho. Credit is granted by both the high school and the college sponsoring the course. See the matrix for a list of courses and the partnering universities. Cost for concurrent credit is the student’s responsibility. See your teacher or school counselor for more information. Courses must be approved by the content area supervisor and the Administrator of Instruction.

Post Secondary Dual Enrollment

Post secondary Dual Enrollment is open to 11th and 12th grade students who have not graduated. District shall grant academic credit to a student who meets the requirements of policy #2121 and successfully completes course work at a post-secondary institution.
